There's only nothing to explain here.
If you report a bug you need to fill in the bug template.
Because this is not enough info to go on.

looks like this is a universal problem across the board. all managers are having the same issue. still need me to submit the bug report?

Bug templates are always needed in order to help managers.
Because other managers where very helpfull by providing the template it has been reported and can be followed here now.
