I won the achievements as listed above. The tournament was "World National Cup Quarter Finals". I played as "Belgium" and won. It was on my 3rd slot. Here is a screenshot from last round (02/28/2017-03/01/2017):

I won this achievement on 03/01/2017. It was in the league of "Turkey" as "Antalyaspor" which is on my 1st slot.

Good day. The first season I played was in Romania with ACS Poli Timisoara. I won the championship and the cup, undefeated. I have also delivered the top scorer, it was Kovalenko. Now, I am playing with Anderlecht, in my second seasons

Those are the achievements which I did not receive:

Great season
Golden Boot
Superman

I am not sure that I can prove the Golden Boot achievement, but maybe you can see it somehow. Here is a photo of my overall results:
